What the form is

Dhaka Metropolitan Police (DMP) asks landlords to collect basic information about their tenants and submit it to the local police station. It was introduced as a security measure, and many landlords now make it part of moving in.

What it usually asks for

  • Name, photo, NID number and contact details
  • Family members and anyone living with you
  • Previous and permanent addresses
  • Workplace or educational institution
  • Domestic workers or drivers, if any
  • An emergency contact

How it is submitted

Usually the landlord submits the form at the local thana, and DMP has also offered ways to submit online. Ask your landlord how they will submit it, and keep a copy of what you provided.

Protect your personal data

  • Give the form only to your landlord or the police, never to unknown agents
  • Don't send NID photos to people you haven't met
  • Ask for a copy of the completed form
  • Cover sensitive numbers in any photo you share before a viewing

If something looks wrong

The real form is for your landlord and the police. If someone asks for money to 'register' you, or wants your documents before you have even seen the flat, be careful and check with the local thana.
